What is Evidence of Coverage (EOC)?

After you’ve compared Medicare plans and enrolled in the one that’s right for you, you’ll want to download your Evidence of Coverage (EOC). This document is your instruction manual for your health insurance plan. Here’s what to know about your EOC.


Why is this document important?

Your EOC is a legal contract between you and your Medicare plan. It is a document that’s made available to you each year by your health insurance provider, usually in October. Your EOC outlines the costs and benefits of your plan that will go into effect on January 1 of the upcoming year.


What information is included in the EOC?

Your EOC gives you a comprehensive roadmap to how your insurance works. It includes details about your benefit information, cost sharing, eligibility, enrollment and how to access health care services. It also lists contact information for regulatory and state departments and your plan’s customer service team.


Where can I find it?

At Sanford Health Plan, we provide your EOC online so it can be easily accessible. Because it’s a lengthy document, we recommend using the search function to scan for key terms.
