Please follow these instructions to request a quote for your small business (1-50 employees) in South Dakota, North Dakota and select counties in Minnesota and Iowa. If you are an agent filling out this form on behalf of your client, please indicate your own contact information to ensure the quote comes to you.

1. Download our template (Excel) to enter your employees' information.

2. Upload your completed template into the request form on this page. The information you provide will be used to generate pricing for each employee that will be covered. 

3. Upon submission, expect to hear back from our team within one business day with your personalized quote.
