Align powered by Sanford Health Plan will offer a new Dual Eligible Special Needs Plan (D-SNP) for eligible beneficiaries beginning Jan. 1, 2025, in Burleigh, Cass, and Morton counties in North Dakota.

Detailed information about the plan and its benefits to those qualifying for both Medicare and Medicaid will be available beginning Oct. 1, 2024. Members may enroll beginning Oct. 15, 2024.

Align powered by Sanford Health Plan is a HMO, PPO plan with a Medicare contract. Our D-SNPs also have contracts with State Medicaid programs. Enrollment in our plans depends on contract renewal.
