Change Your Weigh Diabetes Prevention Program is a research-based program that is a part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention. The program focuses on making small, achievable lifestyle changes to reduce the risk for developing type 2 diabetes. This program helps develop habits for better long-term health.
Meet weekly for the first four months for a total of sixteen sessions, then monthly for the next eight months.
Track food intake, physical activity and weight every week, with a goal to lose 5% or more of your body weight.
Connect with others who are also working on improving their health and taking action on changes.
Find support to work through challenges from your class and individually from your coach
One in three adults has prediabetes and many do not know it. Take the risk test to see if you are at risk for developing prediabetes.
To participate in the Diabetes Prevention Program, individuals must have at least one of the following qualifications:
If you have already been diagnosed by your physician with type 1 or type 2 diabetes, you do not qualify for this prevention program. Please email us at partnersinprevention@sanfordhealth.org to learn more about our diabetes management program.
Online Class
The online program meets using the Webex platform that is accessible using a desktop computer, laptop, smartphone or tablet. A coach facilitates an engaging discussion with participants during the live class. Participants will receive support from their lifestyle coach and fellow classmates who are working on similar goals. All attendees will have access to a guide with educational information related to each session topic. Sanford Health Plan members that are eligible to participate in the program can do so at no cost as part of their wellness benefits.
